Billing terms and conditions

By filling out and submitting the tick testing form you are agreeing to allow ECO Laboratory to bill your credit card for the amount associated with the requested services.  This is a one-time charge that will occur without any prior notification and will show up on your credit card statement.  You are not authorizing any other additional charges outside of this one transaction.  A receipt for this transaction will be sent to you via email.
